Banquet Reservation Form

The Loveland Show Choirs end-of-year banquet will be held at the Oasis Conference Center (902 Loveland-Miamiville Rd.) on Monday, May 11, 2026, at 6:30 p.m.  The cost is $25.00 for showchoir members and $25.00 for family members.

Please fill out the bottom portion of this form and turn it in with payment no later than April 27th.

Show Choir REQUIRED STUDENT Forms

In order to participate in the competition season, Loveland Show Choir members need to have completed the first two forms.

  1. 2026 Travel Permission Slip

  2. Parent/Student Permission Slip

  3. Permission to Dispense Medication

    (as necessary but required at the beginning of the season)

  4. Overnight OTC Permission to Dispense Form

    (as necessary but required at the beginning of the season)
